Innovative Community Participation

At Valour Community Care, our innovative community participation program is designed to support NDIS participants to build the skills, confidence, and resilience needed to actively participate in their community. As a Veteran-Owned and Nurse-Led provider, we deliver capacity-building supports that focus on long-term independence, wellbeing, and social inclusion.

We proudly support participants across Brisbane North and the Greater Brisbane region, working alongside individuals in real community settings to help them achieve meaningful and sustainable outcomes.

A Capacity-Building, Community-Based Approach

Our experienced team works side-by-side with participants in their local community, using a practical and strengths-based approach. We focus on expanding opportunities for community participation, skill development, and pathways toward employment or informal supports.

Our goal is to support participants to transition confidently away from ongoing formal supports wherever possible, while remaining connected to their community in ways that are meaningful to them.

What Participants Can Expect Support With?

Each program is tailored to the participant’s individual goals and interests. Supports may include:

  • Understanding the importance of community engagement, relationships, and social contribution for personal wellbeing
  • Building self-awareness, confidence, and self-care skills
  • Identifying personal interests, strengths, and passions and linking these to education or employment pathways
  • Developing emotional awareness, self-regulation, and resilience
  • Improving adaptive social skills, including understanding social norms and expectations
  • Building confidence around independence, self-worth, and identity
  • Navigating community systems, programs, and services
  • Preparation and support to access training, volunteering, or employment opportunities
  • Developing the capacity to seek help and support within the community without ongoing formal assistance

Our nurse-led framework ensures participant wellbeing, safety, and emotional support remain central to all activities.

Additional Community and Family Supports

Where appropriate, participants may also access:

  • Supported weekend getaways with friends or family
  • Use of Valour Community Care vehicles for community and family support activities
    • Wheelchair-accessible transport (up to two wheelchairs)
    • Seating for up to five additional passengers
